Brian Powell, PT, DPT
Physical Therapy & Rehab Director, Jeff Manley Wellness Center in Price
Brian Powell is the current Director of Rehab at Castleview Hospital. Brian was born and raised in Carbon County. He graduated from CEU with an Associate of Science degree, received a Bachelor degree in Pre-Physical Therapy with a certificate in Gerontology and a Minor in Spanish from Utah State University in Logan, Utah. He then went on to get a Doctor of Physical Therapy degree from Arizona School of Health Sciences in Phoenix, Arizona.
Brian has served as a therapist in the area for more than 12 years. He is certified as a Lymphedema Therapist, a Senior Strength Trainer, a Clinical Instructor, and a Clinical Education Coordinator. He enjoys all aspects of patient care but really enjoys wound care. He works full time for the hospital and part time for the home health agencies in town.
Brian loves spending time in the outdoors with his family. He likes to hunt, fish, and play sports. He especially loves to go boating at Lake Powell. He also spends a lot of time serving others in his church callings.
Paul Smith, PT, DPT
Doctor of Physical Therapy, Jeff Manley Wellness Center in Price
Paul mostly grew up in Provo, Utah. He earned his Bachelors of Science in Exercise Science at Utah State University with a minor in Spanish. He received his Doctor of Physical Therapy at the University of Utah.
Paul's career has focused on orthopedics with a special interest in sports medicine. He works in close association with USU Eastern sports teams, helping with rehab needs.
Outside of orthopedics and sports medicine, Paul has provided spine, aquatic, neurological, balance, vestibular, prophylactic (preventative) and ergonomic care. He has special training in "Total Motion Release (TMR)" techniques for establishing better ROM and strength balances.
Paul enjoys playing sports, especially football and soccer. Off the field he likes mountain biking, hiking, camping, fishing, and spending time with his wife and five kids.
Lee J. Nielsen, PT, MSPT
Physical Therapist, Emery Medical Center and Jeff Manley Wellness Center in Price
Lee is a physical therapist at both the Price and Castle Dale offices for Castleview Hospital. He also provides home care physical therapy for the home health agencies in the Carbon/Emery area.
Lee has almost 20 years of experience as a physical therapist including orthopedics, industrial rehab, skilled nursing and home care. He is also certified as a senior strength trainer, a clinical instructor, and ergonomic assessment specialist, a functional capacity evaluator and a lymphedema therapist.
Lee received his Bachelor of Science in Exercise Science/Psychology from Brigham Young University. He earned his Master of Science in Physical Therapy from Texas Women’s University with magna cum laude honors.
Outside of the clinic, Lee enjoys spending time with his wife, five sons, and two daughters. He enjoys the outdoors, camping, and playing the piano.
Amber Williams, PT, DPT, OCS, ATC

Amber grew up in Emery County, Utah. She attended College of Eastern Utah where she obtained an Associates of Science. She then attended Weber State University and received her Bachelors degree in Athletic Training. She then moved on to receive her Doctorate of Physical Therapy from the University of Utah in 2009. Since graduating, Amber has had particular interest and has received additional training in treatment of running injuries and running analysis, women's health, geriatrics, and has become a certified orthopedic therapist. Outside of her career, Amber enjoys being active outdoors with her husband and two sons. They enjoy camping, hiking, mountain biking, and fishing. She enjoys running and yoga to maintain her own fitness.
Kristi Henricksen, PT, DPT
Doctor of Physical Therapy, Jeff Manley Wellness Center in Price
Kristi is a recent graduate of Rocky Mountain University of Health Professions. She received her Doctorate Degree of Physical Therapy in December of 2017. She received her Bachelor's Degree from Utah State University in Human Movement in 2015. Kristi's clinical training emphasized a manual therapy-based approach and management of orthopedic conditions. Her interests are in manual therapy techniques, orthopedic injuries, and pain neuroscience education. She is excited to pursue additional training in pelvic health.
Kristi grew up in Huntington, Utah and loves small town life. In her free time, she enjoys playing volleyball, being outdoors, road tripping with her husband, and riding Utah's great snow on snowboard or snowmobile. Kristi is excited to be working with friends and family of Carbon and Emery County.
Matt Holt, PT, DPT, OCS, CMTPT
Doctor of Physical Therapy, Urgent Care Physical Therapy Clinic in Price
Matt grew up in Price, Utah. He received his bachelor's degree and Doctorate of Physical Therapy from the University of Utah, graduating in 2010.
Since graduation, he has worked in various rehabilitation settings including hospital services, home health care and outpatient clinics. In 2014 he became an Orthopedic Certified Specialist (OCS). He also is a Certified Myofascial Trigger Point Therapist (CMTPT) through Myopain. Matt also provides specialized services related to injury prevention and wellness for corporations as well as individuals.
He is working on implementation of a functional movement and pain reduction program to assist with opioid addiction.
Outside of the clinic, he enjoys spending time with family and being actively engaged in recreational activities.
Johnny Downard, MOTR/L
Occupational Therapist, Jeff Manley Wellness Center in Price, Castleview Hospital inpatient
Johnny is an Occupational Therapist for Castleview Physical Therapy. Johnny sees patients in the hospital for inpatient treatment and outpatient treatment. His main focus of treatment is in hand, wrist and upper extremity rehabilitation.
Johnny graduated from the University of Utah in 2012 with a B.S. in human development before attending Occupational Therapy school at the University of Utah where he graduated in 2016. During my time in OT school, I was able to do a clinical at the University of Utah Orthopedic Hand specialty clinic. After graduating I have further pursued hand specialization by attending the Teton Hand Conference in Jackson, WY and most recently the American Society of Hand Therapy Conference in Dallas, TX.
I specifically chose Occupational Therapy because of the increased focus on the hand and the upper extremity. The hand is an anatomically complex area of the body with a high level of importance for function and independence. I enjoy the challenge that the hand and upper extremity can present, along with restoring function to my patients.
In my free time I enjoy being active and living a healthy lifestyle. I ride motocross, downhill mountain bike, boat, wake surf, and I enjoy spending time with family and friends.
Shelly Elliott M.S. CCC- SLP
Speech Language Pathologist, Jeff Manley Wellness Center in Price, Castleview Hospital
Shelly is the Speech-Language Pathologist for Castleview Hospital. She also provides services through local nursing facilities and home health agencies in the Emery/Carbon area. She specializes in swallowing disorders and treatment, cognitive therapy, language, articulation, stuttering, and voice therapy.
Shelly graduated from Utah State University with her Bachelor's degree in Communication Disorders and Deaf Education with one semester spent at Yokohama National University in Yokohama, Japan; she then attended Idaho State University and received her Master's Degree in Speech-Language Pathology.
After working in both Idaho and the Salt Lake City area, Shelly moved to Price in order to work for Castleview. Her husband and young daughter (who is a big fan of the Velociraptor statue) accompanied her. Outside of work Shelly loves eating, cooking, hiking and spending time with her family.
Tiffni Monahan, PTA
Physical Therapist Assistant, Castleview Hospital inpatient
Tiffni is a Physical Therapist assistant, who provides inpatient therapy at Castleview hospital, specializing in wound care.
She received her Associates of Science from CEU in 1991 and Associates of Applied Science in 1994 from Fort Morgan College in Fort Morgan, Colorado. She has worked in various therapy settings over the last 18 years including home health, outpatient, and inpatient. Tiffni is married to Dr. Dan Monahan and enjoys skiing with her husband on the weekends during the winter months.
Monique Tabone, PTA
Physical Therapist Assistant, Carbon School District
Monique Tabone grew up in Carbon County and graduated from Carbon High School and CEU. She received her Physical Therapy Assistant Degree from Michiana College in South Bend, Indiana. She has worked in various settings including: outpatient, inpatient, aquatic therapy, skilled nursing/rehab, and home health. Monique currently provides physical therapy to the students in the Carbon School District.
Monique has recently relocated back to the area and is excited to be home and closer to family. Monique enjoys spending time with her 3 kids and supporting them in their activities.
Amber Timothy, L.M.T.
Massage Therapist, Jeff Manley Wellness Center in Price
Amber was born and raised in Heber City, Utah. She attended college at College of Eastern Utah from 1997-1999, where she played volleyball and went on to receive her associates degree in science.
Amber has always had a love for massage and knew it was what she wanted to do in life. She has had experience in both spa and medical settings. She has focused on and loves deep tissue, trigger point, and sports massage.
Amber is very active and enjoys any kind of sports. The loves of her life are her husband and three kids. In their spare time they love boating, camping, going to Lake Powell and Disneyland
