Darian Wilks, OTR/L
Darian is an Occupational Therapist and Physical Therapist Assistant for Castleview Hospital. Her primary focus of treatment is hand, wrist and upper extremity rehabilitation. She works with a variety of other populations such as neurological conditions, feeding and sensory difficulties, pediatrics, and aquatic therapy. She also does Home Health therapy, Early Intervention and School District therapy.
Darian grew up here in Price and graduated from USU-E with her bachelor’s degree in Health Promotion & Education. She also has an associate’s degree as a Physical Therapist Assistant and went on to get her master’s degree in Occupational Therapy from the University of Utah.
She absolutely loves her job and getting to interact with people of all ages and diagnoses and help them return to the things that give their lives meaning. In her spare time, she enjoys spending time with her husband and two young daughters, as well as boating, hiking and playing volleyball.
Amber Williams, PT, DPT, OCS, ATC
Amber Williams is a dedicated physical therapist with more than 15 years of experience. Having a Doctor of Physical Therapy from the University of Utah, she has helped people from all walks of life achieve their goals for functional and physical activity.
She has extensive experience in treating orthopedic injuries initially as an athletic trainer and then as a physical therapist. She went on to become an orthopedic certified specialist in 2015. She particularly likes to work with runners and has additional training in running analysis and rehab. In the past six years she has gone through extensive training to become more specialized in pelvic rehab. This would include treating conditions having to do with bladder and bowel dysfunction, pain and sexual dysfunction. She diligently works to improve her skills in both orthopedics and pelvic health and is currently working towards her dry needling certification.
On a personal note, Amber grew up in Emery County and is a wife and mother of two active boys. She enjoys nearly any kind of outdoor activity including gardening, hiking, snowboarding, mountain biking and paddle boarding. She likes to participate in running, yoga and weightlifting to maintain her physical health. She feels that having a family while being dedicated to her own personal health gives her a greater understanding of how to help her clients achieve their goals while managing the mayhem of life.

Nikell Tamllos, CEP
Nikell has almost seven years of experience in Cardiac Rehab, where she supports patients in their recovery from heart-related events through structured cardiac rehabilitation program. She has a bachelor’s degree in Kinesiology with a minor in Nutrition and has been a Certified Exercise Physiologist since 2019. Nikell is committed to helping patients improve their heart health and adopt healthier lifestyle.
Monique Tabone, PTA
Physical Therapist Assistant
Monique Tabone grew up in Carbon County and graduated from Carbon High School and the College of Eastern Utah. She received her Physical Therapist Assistant Degree from Michiana College in South Bend, Indiana. She has worked in various settings including schools, pediatrics, skilled nursing/rehab, home health, outpatient, inpatient and aquatic therapy. Monique currently enjoys providing physical therapy to the students in the Carbon School District and Pinnacle Canyon Academy. Monique enjoys spending time with her three kids and her extended family.
Paul Smith, DPT
Paul earned his Bachelor of Science in Exercise Science at Utah State University with a minor in Spanish. He received his Doctor of Physical Therapy at the University of Utah.
Paul's career has focused on orthopedics with a special interest in sports medicine. He works in close association with USU Eastern sports teams, helping with rehab needs. He also is a Certified Bike Fitter with "Bike Fitness Coaching" and International Bike Fitting Institute and runs Castleview Bike Fit Studio, serving South Eastern Utah for MTB, road and tri bikes. Paul helps coach the local NICA MTB cross country bike team.
He also received his certificate of achievement for completing and passing the course of Vestibular Rehabilitation: Competency-Based Course, allowing him to be more specialized in vestibular rehabilitation.
Outside of orthopedics and sports medicine, Paul has provided spine, aquatic, neurological, balance, prophylactic (preventative) and ergonomic rehabilitation.
Paul enjoys playing sports, especially football and soccer. Off the field he likes mountain biking, hiking, camping, fishing and spending time with his wife and five kids.
Chelsie Hreinson, MS OTR/L, CLT
Occupational Therapist
Chelsie is an occupational therapist with the hospital who works with a large variety of demographics, including pediatric development, sensory processing, pediatric behaviors, neurological issues, upper extremities, aquatic therapy, school therapy and lymphedema. Chelsie is lymphedema-certified.
She is a native to the area and has been an occupational therapist for 13 years, working in many settings, including long-term care, acute care and outpatient, to name a few. She attended CEU, Provo college and graduated from the University of Utah with her bachelor's degree in Behavioral Science and Health and then her Master's in Occupational Therapy in 2011. She understands the complexities behind healing and takes a more holistic approach to the process. She loves getting to know her clients well and hopes they feel like family as they work together.
Chelsie is a very devoted mother of four children and wife to her husband, John. She loves camping in the summer, playing with her kids, playing the piano and spending time with family.
Shelly Elliott, M.S., CCC- SLP
Speech Language Pathologist, Jeff Manley Wellness Center in Price, Castleview Hospital
Shelly is the Speech-Language Pathologist for Castleview Hospital. She also provides services through local nursing facilities and home health agencies in the Emery/Carbon area. She specializes in swallowing disorders and treatment, cognitive therapy, language, articulation, stuttering and voice therapy.
Shelly graduated from Utah State University with her bachelor’s degree in Communication Disorders and Deaf Education with one semester spent at Yokohama National University in Yokohama, Japan; she then attended Idaho State University and received her master's degree in Speech-Language Pathology.
Outside of work Shelly loves to care for her large flock of chickens and garden and spend time with her family.
Kristi Henricksen, PT, DPT, OCS
Doctor of Physical Therapy, Jeff Manley Wellness Center in Price
Kristi graduated from Utah State University in 2015 where she earned her Bachelor of Science in Human Movement. She received her Doctor of Physical Therapy degree in 2017 from Rocky Mountain University of Health Professions. Kristi became a certified Orthopedic Clinical Specialist in 2022 through the American Board of Physical Therapy Specialists.
As part of her clinical education, Kristi spent a summer in interior Alaska emphasizing a manual therapy and pain neuroscience approach for the management of orthopedic conditions. Kristi enjoys working with post-surgical conditions including total joint replacements, rotator cuff repairs and ACL repairs. She has experience treating torticollis and plagiocephaly for the pediatric population. Kristi also works with a wide variety of patient populations and disorders including sport injuries, Multiple Sclerosis, post-stroke rehab, osteoarthritis conditions, aquatic therapy, prehab, balance and fall prevention, and work-hardening programs.
Kristi grew up in Huntington, Utah, and loves small-town life. In her free time, she enjoys playing volleyball, being outdoors, road tripping with her husband and riding Utah's great snow on snowboard or snowmobile. Kristi is excited to be working with friends and family of Carbon and Emery County.
Brian Powell, PT, DPT
Physical Therapy & Rehab Director, Jeff Manley Wellness Center in Price
Brian Powell is the current Director of Rehab at Castleview Hospital. Brian was born and raised in Carbon County. He graduated from CEU with an Associate of Science degree, received a Bachelor degree in Pre-Physical Therapy with a certificate in Gerontology and a Minor in Spanish from Utah State University in Logan, Utah. He then went on to get a Doctor of Physical Therapy degree from Arizona School of Health Sciences in Phoenix, Arizona.
Brian has served as a therapist in the area for more than 12 years. He is certified as a Lymphedema Therapist, a Senior Strength Trainer, a Clinical Instructor, and a Clinical Education Coordinator. He enjoys all aspects of patient care but really enjoys wound care. He works full time for the hospital and part time for the home health agencies in town.
Brian loves spending time in the outdoors with his family. He likes to hunt, fish, and play sports. He especially loves to go boating at Lake Powell. He also spends a lot of time serving others in his church callings.
Lee J. Nielsen, PT, MSPT
Physical Therapist, Emery Medical Center and Jeff Manley Wellness Center in Price
Lee is a physical therapist at both the Price and Castle Dale offices for Castleview Hospital. He also provides home care physical therapy for the home health agencies in the Carbon/Emery area.
Lee has almost 20 years of experience as a physical therapist including orthopedics, industrial rehab, skilled nursing and home care. He is also certified as a senior strength trainer, a clinical instructor, and ergonomic assessment specialist, a functional capacity evaluator and a lymphedema therapist.
Lee received his Bachelor of Science in Exercise Science/Psychology from Brigham Young University. He earned his Master of Science in Physical Therapy from Texas Women’s University with magna cum laude honors.
Outside of the clinic, Lee enjoys spending time with his wife, five sons, and two daughters. He enjoys the outdoors, camping, and playing the piano.
Matt Holt, PT, DPT, OCS, CMTPT
Doctor of Physical Therapy, Urgent Care Physical Therapy Clinic in Price
Matt grew up in Price, Utah. He received his bachelor's degree and Doctorate of Physical Therapy from the University of Utah, graduating in 2010.
Since graduation, he has worked in various rehabilitation settings including hospital services, home health care and outpatient clinics. In 2014 he became an Orthopedic Certified Specialist (OCS). He also is a Certified Myofascial Trigger Point Therapist (CMTPT) through Myopain. Matt also provides specialized services related to injury prevention and wellness for corporations as well as individuals.
He is working on implementation of a functional movement and pain reduction program to assist with opioid addiction.
Outside of the clinic, he enjoys spending time with family and being actively engaged in recreational activities.
Tiffni Monahan, PTA
Physical Therapist Assistant, Castleview Hospital inpatient
Tiffni is a Physical Therapist assistant, who provides inpatient therapy at Castleview hospital, specializing in wound care.
She received her Associates of Science from CEU in 1991 and Associates of Applied Science in 1994 from Fort Morgan College in Fort Morgan, Colorado. She has worked in various therapy settings over the last 18 years including home health, outpatient, and inpatient. Tiffni is married to Dr. Dan Monahan and enjoys skiing with her husband on the weekends during the winter months.
Amber Timothy, L.M.T.
Massage Therapist, Jeff Manley Wellness Center in Price
Amber was born and raised in Heber City, Utah. She attended college at College of Eastern Utah from 1997-1999, where she played volleyball and went on to receive her associates degree in science.
Amber has always had a love for massage and knew it was what she wanted to do in life. She has had experience in both spa and medical settings. She has focused on and loves deep tissue, trigger point, and sports massage.
Amber is very active and enjoys any kind of sports. The loves of her life are her husband and three kids. In their spare time they love boating, camping, going to Lake Powell and Disneyland
